Community Health and Education Services (CHES) offers specialized nursing support services for clients with Cerebral Palsy, spina bifida, and those on the autism spectrum.
The team works with the client’s Primary Care Provider to design a customized plan of care that supports daily activities, events, and all other childhood moments.
The group provides nursing consultations and nursing case management and delegation solutions, all available to Self-Directed Services (SDS) members.
CHES uses the Health Risk Screening Tool (HRST) to identify or review clients’ health care level.
The group also drafts healthcare protocols, establishes ways for communication with healthcare providers, and finds health-related resources available in their communities.
Its team makes nursing and self-administration assessments and oversees the training and work of the staff authorized to implement the tasks detailed in the client’s care plan.
CHES also provides personal care services for older children. The nurses can assist individuals with everyday tasks such as eating, bathing, or light housework.
